I called a number of private medical institutions, with a question about being a psychiatrist and a psychiatrist-narcologist, they all said: “... go to the dispensaries, then with their certificates come to us, we’ll process it within 10 minutes.” There is no choice, you will have to go to dispensaries.

I first chose a psychoneurological dispensary, since in the Southern District I had to go to “Psychiatric Hospital No. 14 Branch No. 2” (Moscow, Proletarsky Prospekt, 4), I went by car, and if on foot, it is not far from the metro Kashirskoe.

Actually, I saw an unremarkable building with a dispensary sign, went in, there were two registration windows inside on the right, after reading what was written on the windows, I stood in line (about 7 people) at the second window. I waited in line for about 20 minutes, they weren’t particularly fast, I said that I needed a certificate to replace my driver’s license and category, and gave me my passport.

In response, they gave me a receipt that needs to be paid, an agreement and a card in which I need to fill out the data. Fortunately, the savings bank is nearby (about 10 meters), I went and paid the receipt, with the commission it came out to 714 rubles. Filled out the details on the card and got in line at the office. I waited another 20 minutes, then went into the office.

In the office, a nurse and a doctor will start asking questions, something like: “Are you driving in reverse on the Moscow Ring Road,” and how do you feel in general, for a total of 3 minutes and give you a certificate. I give you the contract, payment receipt and card. Fill in the data according to the sample. We go to the office and get in line, I was lucky here, there were only 3 people in front of me.

Here they tortured me a little, forced me to show my hands, teeth, move my eyes, suggestive questions, in total for about five minutes and gave me a certificate, it was also necessary to affix a stamp at the registry, without a queue.

While I was sitting in line, I asked others how the examination process was going, some I simply asked, and some, apparently especially suspicious, were forced to take tests. So if you have sinned somewhere with your body, it is better to wait two weeks before visiting a dispensary.

Other doctors

The rest of the doctors can be seen in private medical institutions with licenses. Here I have already chosen a private clinic nearby and which has the right to issue such certificates. You can check the entire institution on the Open Data portal.

I came to them with certificates from dispensaries, paid 900 rubles, filled out a contract (you could have gone to your own clinic, but you yourself understand the queues, and you still have to pay for it).

They scanned the certificates, sent them to the offices, after 5 minutes I returned to the reception and handed over the completed certificate. A photo is not required, but if you want, they can paste it in.

I also took a copy of the license from them, they issued it without any problems, you know that we live in Russia and if the inspector doesn’t like something, there’s no particular desire to go to them again (the inspector didn’t even ask for a license).

Actually, that’s all, in half a day a certificate was received and 2,300 rubles were spent.

Step 2. Registration on the State Services portal and identity confirmation

A little about the advantages of sending an application through the State Services portal:

  • you don’t need to come to get a ticket, you can choose a convenient time without leaving your home;
  • When paying state fees non-cash through the portal, we receive a 30% discount.

There is such a wonderful portal as State Services, if you are already registered there, you can immediately start filling out the application. If you are there for the first time, then you need to register. The order is:

  • go on the website;
  • register an account using a phone number or email;
  • Fill out your profile with personal data – passport, TIN, SNILS, and so on. You will see the full list in your personal account;
  • and confirm your identity.

I won’t tell you more about how to register, everything is simple there. I'll tell you a little about identity verification. Actually, after registration you are given an access level, there are three in total:

  • simplified;
  • standard;
  • confirmed.

In the first case, you will not be able to draw up documents; you will only be able to use the information section. The second level provides more opportunities, but again not all. Well, the latter provides access to a full list of services, which is what we need.

There are three ways to confirm your identity:

  • personal visit to the CO (service center);
  • registered letter by mail;
  • electronic signature.

I chose the first one, fortunately there are many post offices in Moscow, but not all of them confirm the truth.

We select a branch that is convenient for you from the list on the portal, go there with your passport, and say that we want to confirm your identity on the State Services portal. You will be given an application, fill it out.

Step 3. Submitting an application through the State Services portal
